It has become challenging for humans to beat computer systems and logics formulated behind them. Developed by humans for the excellence of processes in day to day life and for the care of population.

There are four ways of approaching Artificial Intelligence:

  1. Thinking humanly – cognitive science
  2. Thinking rationally – logic
  3. Acting humanly – turing tesing
  4. Acting rationally – rational agent

Thinking or acting deals with thinking processes or behavior, while humanly or rationally means humanly or to do the right things.

The AI approach has been taken to such a level where robots and software are communicating with human in their language. In fact the AI’s approach is to act rationally and to build rational agents, and these agents are nothing but the robots and software.

Gestation of Artificial Intelligence (AI) was from 1943-1955, where the AI developed in a cogitative process. First work on AI was started by Warren McCulloch and Walter Pitts in 1943, where the duo developed and worked on artificial neuron model where the neurons where either on or off to function.

“How AI has become an Industry” and entire processes has been given a cobwebbed connection to increase the production and output by many folds. None of the major processes in industries are free from software applications and machines that are guided by codes.

Challenges to be handled in future:

Though this ever growing technology is helping each and every one of us and at the same time it is keeping humans away from the nature and that’s where the natural processes are evolving and leading to a world of so many unexpected disorders, health issues.

Next Generation Sequencing (NGS)- breaking the human genome code

So far now, it has become possible to sequence anyone’s genome cheaper and quickly with the help of latest technologies. Researchers have started gaining much into discovering the meaning of individual genomes. Though still the sequencing has not come down to an affordable price but has been sponsored mostly by the stake holders of the research projects to get into the main stream of the genomics worlds. And it has been highlighted that the individual genome sequencing would help track the rare deformities and anomalies in the gene line of human and can be engineered to treat or suppress the genes responsible for the cause.

Evolution in technology:

Things have evolved to an unimaginable ground and would continue evolving. Folks living a gadget life from the time they come out and goes back to their bed. Evolution has almost made everyone hooked to gadgets whether it’s handy or fixed. Things have changed where lot of thinking and effort was required but now solutions are available right there in hands and with least to no efforts at all.

Technology has made our life so simple to a damn complicated one at the same time. Well, it’s you or me; we can’t imagine a minute without android phone or may a system and a fast life, fast food & fast*. And I am enjoying like others a handy gadget in my hand and a laptop from day to night.

It is not wrong to say, that we are living an entirely hacked life, where we have to have the things that are available in market, whether it is produced by using chemicals or other harmful ways. We can’t stay away from the exposure of radiations and other harmful elements that are around us every moment. So in short, we gotta inhale smoke and eat crack food and live this way. There are people who are involved in hacking human lives to the worst to fucking death and it’s unavoidable in many circumstances for many of us.

Our body systems are in need of such evolved technology now. Sooner or later one gotta go have some transplantation, surgery to live and extend the life expectancy. Need not to worry, you have got the replacement too for every failed part of your body at some part of this world and you gotta find when you need it. One will get every gene when required soon by the time one will need. Human system is like computers now or like any other machine that can be assembled the best by choice (gene responsible for every aspect in our body and behavior) from around the world and get a customized baby out of it. It’s not as simple as sounds. Results could be horribly upside down in many cases.

Science has developed so far and with the help of available techniques and procedures, it has become quite easy to knock down many disease conditions and other congenital conditions occurring in human lives. Techniques developed till now have really worked well in improving lives. There are many techniques that are being into developing stage and enthusiasts are working hard on it to make it possible to pin down every possible disease and other ailment conditions. One such congenital condition is called, cleft lip and cleft palate. It affects 1 in every 600 children born worldwide (1) and is one of the most commonly occurred congenital deformities. It has been explained and shown the particular gene playing role in sole or in association with other factors playing role in the formation of non-syndromic cleft lip and/or cleft palate. GMO – An alternative to deal with malnutrition in developing countries.

It is not wrong to say, that GM products and crops and what so ever till now has been developed out of it after a 3 decade long research has given good and bad results.

In most of the cases it is has given better results and is quite good for the world to tackle the serious problem of hunger in developing counties.

Anti-GMO groups have proclaimed many of their points are been seen misguiding the population by citing the GMOs to be:

  • Unsafe, unhealthy
  • Increase level of chemical usage
  • Carcinogenic
  • Ability to harm the environment
  • Corporate control on agriculture business
  • Reduction of biological diversity
  • Dangerous for consumption
  • Problem of causing superweeds etc etc.

The force working for the development and productions of successful GMOs have enough proves and trial records for its safety, less utilization of chemical, harmful for the environment, not dangerous for consumption etc. I guess researchers have worked enough on bring this GMOs and are indeed good and being modified to sustain for long for the coming generations. This way they have been able to save plants which are on the verge of quenching.

The main concern behind writing this project is to understand hospital management in depth together with knowledge of cleft lip and cleft palate deformity. As cleft deformity is one of the most pandemic deformities found with other diseases. But it has not been given much attention as governments and other healthcare organizations have given in preventing and understanding other diseases and deformities.

Diseases and other ailments are on high these days, technology is changing at an unimaginable speed. It has changed people’s live style and other activities drastically. New generation is least bothered about the healthcare issues and they accept what comes in front of them without giving any second thought about affect of their action or routine in future.

Simultaneously to know more about the cleft lip and palate deformity and its consequences at whole. What are the types of cleft lip and cleft palate that is affecting children worldwide? Its consequences could sometimes results into the death also, if not treated and directed for safety measures at right time.

  1. What is cleft lip and cleft palate

Cleft lip and cleft palate

Cleft lip, also known as cheiloschisis, and cleft palate, also known as palatoschisis, are types of abnormal development of the face during the development of the fetus. They are types of congenital deformities. It can occur as cleft lip or cleft palate and together as cleft lip and cleft palate.

Cleft lip

This is when the deformity affects the lip only not the palate. Cleft may be small and results in an indentation on the lip. In some cases, it does not limit to the lip, it may go all the way to the nose. A slight cleft is known as a partial or incomplete cleft. A complete cleft goes all the way to the nose.

Cleft palate

This is when the two parts of the skull which form the hard palate are not fused together, the soft palate also has a gap (cleft). A complete cleft palate may results into a gap in jaw, while an incomplete cleft palate looks like a hole in the roof of the mouth (cleft soft palate)

Cleft lip or palate defects are found to be more common in Caucasian and Asian babies, compared to babies of African descent. Just cleft palate is more common in females, while the combination of cleft lip and palate is more common in males. Half of all children born with such defects have the combination cleft lip and palate, while the rest are evenly divided.

Most cleft can be categorized into three following groups:

  1. cleft lip without a cleft palate
  2. cleft palate without a cleft lip
  3. cleft lip and cleft palate together

It is important to know that the risk of occurrence of Isolated Cleft Palate (ICP) and Cleft Lip / Cleft Palate (CL/CP) is very different (refer table 1) with Cleft Lip / Cleft Palate being far more common. Moreover multiple malformations are far more likely in ICP (42%) as compared to CL/CP where multiple malformations syndromes occur only in 14%.

The most common associated disorder with ICP is the Pierre Robin variant while the most frequently occurring syndrome is Stickler syndrome (17 % of syndromic clefts, autosomal dominant, severe myopia, retinal detachment, and glaucoma).

  • Physical problems & symptoms

Cleft deformity is not responsible for only one problem, but it brings a pack of problems for the one who born with cleft deformities. After baby born with cleft, it requires lots of care from their parents and doctors, in terms of feeding guidelines and lots of the trainings and help to their family to bring their child up with no infection and malnutrition. So it becomes hard job to take extra care for such babies.

  • Awareness about this congenital deformity

There is very poor awareness about cleft deformity among people or in communities. It’s a fact that it’s one of the dominating deformity conditions that is affecting every 1 child born in 600 annually. It is fourth most common birth defect in the U.S. Cleft occurs more often in children of Asian, Latino or Native American descent. Compared with girls, twice as many boys have a cleft lip, both with or without a cleft palate. However, compared with boys, twice as many girls have cleft palate without a cleft lip.

It is right that conditions like cancer, tuberculosis, AIDs etc. are also playing havoc in terms of death around the world. One such deformity is called cleft lip and cleft palate that is affecting many children around the world and leading to severe infections and even death. It has been evidenced that even government is least bothered about such conditions and this deformity has not fetched much attention for the prevention and in terms of safety. Somehow NGOs and centers are able to provide free of surgeries to the needy people from our society. This has helped a lot in re integrating them into their life style.

  • Etiology

The exact cause for cleft lip and cleft palate is not known. But it has been asserted with the help of several studies done in support to answer the real cause of these deformities that, environmental, genetic chromosomal factors may be responsible for the occurrence of this anomaly with all its variations.

It has been thoroughly studied on animals too, and results have posited that experiments have developed clefting in the offspring by varying diets, drug exposures, radiation and vitamin dosages.

It has been postulated that a protective influence in the prevention of cleft lip and cleft palate formation with the folic acid supplementation before and during the first trimester is now being into effective step. Families known to be at the risk for clefting have also been used for studies.

  • Types of cleft Lip & Palate

Cleft lip types

  • Forme fruste unilateral cleft lip
    A subtle cleft on one side of the upper lip, which may appear as a small indentation.
  • Incomplete unilateral cleft lip
    A cleft on one side of the upper lip, which does not extend into the nose.
  • Complete unilateral cleft lip
    A cleft on one side of the upper lip, which extends into the nose.
  • Incomplete bilateral cleft lip
    Clefts on both sides of the upper lip, not extending to the nose.
  • Complete bilateral cleft lip
    Clefts on both sides of the upper lip, extending into the nose.

Cleft palate types

  • Incomplete cleft palate
    A cleft in the back of the mouth in the soft palate.
  • Complete cleft palate
    A cleft affecting the hard and soft parts of the palate. The mouth and nose cavities are exposed to each other.
  • Submucous cleft palate
    A cleft involving the hard and/or soft palate, covered by the mucous membrane lining the roof of the mouth. May be difficult to visualize.
  • Causes for the deformity

There is no fixed factor for such deformities and in most of the cases, the cause for the cleft lip and cleft palate is unknown. But many factors are there that gives rise to a cleft child during fetus formation. Researchers have been trying to find the exact cause of the deformity in individual, but still it has not been proved for one particular cause. As there are several factors and conditions that plays role in the formation of fetus with cleft lip or palate or with both. Most scientists supports cleft are due to the combination of genetics and environmental factors. There appears to be a greater probability of appearing cleft in child if, sibling, parent or relative has had the problem.

Some factors that play role in the formation of cleft child are as follows:

  • Hereditary
  • Environmental
  • Consanguineous marriages
  • Syndromic cases
  • Smoking and alcohol abuse
  • Use of drugs or steroids during pregnancy or before
  • Deficiency of folic acid in women’s diet during pregnancy

Exposure to some viruses or chemicals while the fetus is developing in the womb.

DEVELOPMENTAL BIOLOGY OF LIP AND PALATE

Our head is one of the complex systems that develop with the help of number of complex events and processes that underline normal face. The chances for abnormal facial appearance and other facial deformities are many in numbers that could result in new born due to many reasons. And many a times that reason can not be find and finding of the real cause becomes a challenge for counselors for such families. Since the genes responsible for the formation of lip initiates its process and with the help of proliferating cells in respective zone of the body, the particular organ internal or external takes place. The development of the lip takes place before the palate formation, like lip starts forming in first trimester that is at the 4-5th week of gestation period followed by the formation and adhesion process of the palate (upper roof of the mouth) in 7th week. So, keeping this complex process in mind, couple should and must mind not consuming tobacco, alcohol and some other hormonal drugs that might hamper this process.

It is very important for couples to see geneticist for their counseling before and after planning for the offspring. There could be the chances of inheriting some deformities or other abnormal conditions from parents to offspring. Sometimes it is not possible to avert a strike but sometimes by taking care of nutritional diets, upto some extent some of the conditions and deformities can be prevented.
